Rusty Russell c503232cde common: use bitcoin_outpoint.
I started pulling this thread, and the entire codebase got unravelled.

We actually do need this for dualopend's, specifically for returning
errors to open_channel2

niftynei 42a38b9808 channel_id: add v2 channel_id derivtion helper
v2 of channel open uses the channel revocation basepoints to calculate
the channel_id, instead of the funding_txid + outnum

Moving away from the funding_txid opens the way for splicing + rbf

Rusty Russell fda5f0b427 common/channel_id: move channel_id into its own file.
The definition was in wire/wire.h, and helper functions in fromwire.c!
